When you submit the form on this site, it collects your ZIP code, name, age, phone number, how you prefer to be reached, when you need coverage, a household income band and, if you choose to give it, an email address and the situation you describe. It is used only to prepare and send health coverage options and to contact you about them.
The information is used to work out which coverage options may be available to you and to follow up by text, phone call or email, as you chose on the form. It is not sold to unrelated third parties.

Submissions travel over an encrypted connection to the form service (Netlify) and are kept only as long as needed to handle your request. The site uses Google Analytics to count visits; no form contents are sent to it.
